Praxis Health is seeking a Family Medicine Nurse Practitioner to join our primary care team in La Grande, Oregon. You'll practice alongside two physicians and three experienced Advanced Practice Providers, with an integrated LCSW and two Community Health Workers who help support the mental health and broader needs of our patients.
